Plan Ahead: Your Decisions Matter

This program helps individuals understand the importance of planning ahead for future health care and financial decisions.

Participants will learn about key documents such as Advance Directives, POLST (Practitioner Orders for Life-Sustaining Treatment), Power of Attorney, and Last Will and Testament in this program.
The presentation also encourages reflection on personal values, choosing a trusted healthcare proxy, and having important conversations with loved ones before a crisis occurs.
The goal is to help individuals stay in control of their decisions and provide peace of mind for themselves and their families.
